ADvantage Case Manager

As a Case Manager, you will support our residents while collaborating with a team that values purpose, dedication, and teamwork. This is a remote position serving the Tulsa metro area. Daily travel in the greater Tulsa area is required. This position is responsible for the ADvantage case management process, which includes admission/intake, assessment/reassessment, service planning, service plan implementation, monitoring/evaluation, and member discharge. The roles assumed by the ADvantage case manager within the long-term care service delivery system are service coordinator, ADvantage member advocate, ADvantage member consultant, and gatekeeper.

General Responsibilities

  • Intake, Screening, & Referral – Establishes procedures for information requests regarding community long-term care options.
  • Access – Facilities access to ADvantage case management services for ADvantage members.
  • Admission – Admits only those ADvantage members determined eligible and referred to the Case Management.
  • Reviews the comprehensive assessment, and/or completes reassessment, as needed, to identify ADvantage member needs, goals, abilities, resources, and supports as the basis for service planning.
  • Facilitate and provide ongoing education to ADvantage members as needed to empower them to be informed and active ADvantage members.
  • Ensures coordination of available community resources and informal support, for which the ADvantage member is eligible and/or has needs.
  • Submits a comprehensive, written service plan for each ADvantage member that was developed using the interdisciplinary team process; that establishes service goals to meet identified ADvantage member needs and promote independence, that uses the most effective and cost efficient formal and informal resources, and that clearly defines implementation responsibilities.
  • Accepts direction and feedback from the ADvantage Case Manager Supervisor in a positive, constructive manner.
  • Implements the approved service plan.
  • Continuously monitors the delivery of services, evaluates member outcomes, and makes revisions as indicated.
  • Uses risk-management mechanisms to address high-risk situations and special needs of ADvantage members.
  • Modifies the service plan based on the status of the Advantage member without disruption of services or threat to the ADvantage member’s health and safety.
  • Reviews and monitors direct service provider emergency procedures to protect the health and safety of the Advantage member.
  • Collaborates with the staff Registered Nurse regarding medically complex members.
  • Ensures confidentiality in accordance with all applicable laws and regulations.
  • Maintains appropriate and adequate records to document activity in the performance of responsibilities as a component of the service delivery system.
  • Participates in all other job duties as requested and/or assigned by the Advantage Case Management Supervisor or Director.
  • Participation in all training and continuing education as required.

General Qualifications

  • Bachelor’s degree in social sciences, nursing, or a health-care related field
  • At least two (2) years’ professional experience with aging and/or disability populations or programs, or equivalent training or certification
  • Must have completed or be in the process of completing the ADvantage case management training program. Training and certification are available.
  • Willing to learn and follow directions

Physical Working Requirements

  • Must possess sight and hearing, or use prosthetics that enable these senses to function adequately to meet the position requirements.
  • Must be able to bend, stretch, reach, lift, or move up to 25 pounds at a time, turn, and walk. Occasionally lift/move up to 50 pounds.
  • Local travel is required.
