LPN Admissions Coordinator

POSITION SUMMARY:

The Admissions Coordinator is responsible for initiating and fostering positive community relationships while supporting the admission process for all new residents and skilled guests. This role reviews referrals, conducts initial pre-admission evaluations, coordinates admission documentation, and works closely with referral sources, residents, families, and healthcare professionals to help ensure a smooth transition into the community.

The ideal candidate is organized, responsive, clinically knowledgeable, and skilled at building strong professional relationships.

 

KEY RESPONSIBILITIES:

  • Review documentation for resident and skilled guest referrals and perform initial pre-admission evaluations
  • Maintain and build positive rapport with hospital discharge planners and other referral organizations
  • Respond to all incoming referrals in a timely manner
  • Obtain physician orders and other pertinent admission documentation
  • Enter initial medication and treatment orders
  • Assist with resident transfers and discharges
  • Complete PASSR documentation for all new admissions
  • Complete insurance prior authorizations and continued stay requests
  • Support the admissions process to help ensure accurate and timely coordination of care
  • Perform other duties as assigned by the Director of Case Management\

 

QUALIFICATIONS:

  • Must be a Licensed Practical Nurse with at least two years of experience in long-term care
  • Must enjoy working with residents, families, and other healthcare professionals
  • Must be dependable and willing to accept responsibility
  • Ability to read, write, and speak English
  • Strong communication, organization, and relationship-building skills

 

WORKING CONDITIONS:

  • Must enjoy working with residents, families, and other healthcare professionals.
  • Must possess sight/hearing senses or use prosthetics that will enable these senses to function adequately to meet the position requirements.
  • Must be able to squat, bend, stretch, walk, and turn.
  • Must be able to lift up to 25 pounds.
